The Georgia Department of Behavioral Health and Developmental
Disabilities (DBHDD) is seeking qualified candidates for Behavioral
Health Counselor/Team Leader at Southwestern State Hospital in
Thomasville, GA
This individual acts as a Team Leader working on the Developmental
Disabilities unit. The incumbent will serve as a Mental Health and Mental
Retardation Team Leader and as the Qualified Mental Retardation Professional
(QMRP). S/he will chair the Interdisciplinary Team in an ICF/MR setting
and is responsible for treatment plan development that includes skill
development in the areas of Activities of Daily Living (ADL) and prevocational.
S/he ensures service delivery to individuals according to the treatment
plan. As Team Leader, this individual will have managerial
responsibilities for direct care staff.
Job Responsibilities & Performance Standards:
- Performs job
responsibilities with minimal supervision
- Serves as job
sxpert or organization resource in assigned areas
- Act as client
advocate in order to coordinate required services or to resolve emergency
problems in crisis situations.
- Collect
information about clients through interviews, observation, and tests
- Counsel clients
in individual and group sessions to help them overcome dependencies, recover
from illness, and adjust to live
- Develop and
implement treatment plans based on clinical experience and knowledge
- Evaluate
client's physical or mental condition based on review of client
information
- Maintain
confidentiality of records relating to client's treatment
- May supervise
professional staff and/or support level staff
- Monitor,
evaluate, and record client progress according to measurable goals
described in treatment and care plan
- Prepare and
maintain all required treatment records and reports

Bachelors degree in related field AND Four years of related
experience;
OR Masters
degree AND Two years related experience;
OR One year
at the lower level or equivalent position.
|
- Masters degree
from an accredited university in one of the following fields:
- Psychology
- Special
Education
- Social Work
- Experience in an
ICF/MR facility
- Two years of
supervisory experience
- Experience
writing programs and gathering data
- Experience
working with persons with developmental disabilities and in a hospital
setting
- Proficiency in
Microsoft Word and Excel

This position is a pay grade 15 on the statewide salary plan.

This position is located at Southwestern State Hospital in Thomasville, Georgia. More than a century ago, Thomasville was celebrated as one of the most fashionable places in the world to visit.
