Treatment Team Facilitator
Georgia Regional Hospital at Savannah is seeking qualified candidates for the position of Treatment Team Facilitator with AMH Treatment Teams. This full-time position is located on Acute Care Unit 6.
Manages the development and implementation of individual interdisciplinary treatment plans for adults with developmental and/or mental health disabilities. Provides supervision to direct care staff on assigned living area as required. Writes and develops individualized recovery plans and coordinates with interdisciplinary treatment team members to ensure the timely completion of treatment plans consistent with all relevant quality assurance standards. Conducts staff training on essential elements of individualized recovery care planning and team standards. Leads inter-disciplinary treatment team meetings. Manages patient review schedules to ensure all patient cases are managed according to policy guidelines and consistent with quality standards.
Bachelor's degree in related field AND Four years of related experience OR Masters degree AND Two years related experience OR One year at the lower level or equivalent position Note: Some positions may require certification or licensure.
In addition to the qualifications listed above, preference will be given to those candidates who possess the following: Master's degree in human services, nursing, social work, or psychology.
