The Georgia Department of Behavioral Health and Developmental Disabilities (DBHDD), is seeking qualified candidates for the position of Social Service Coordinator - Lakeside at Georgia Regional Hospital At Savannah (please visit www.garegionalsavannah.com for more information). This position is located at Lakeside Center Child & Adolescent Crisis Stabilization Program in Bloomingdale, GA.

Job Description:

Manages, coordinates and directs the social service activities of a program. Provide persons, families, or vulnerable populations with the psychosocial support needed to cope with chronic, acute, or terminal illnesses, physical or developmental disabilities. Provides patient education and counseling, advises caregivers, and makes necessary referrals for other social services. Supervises and plans work of assigned staff. Job Responsibilities & Performance Standards: 1. Supervises and plans work of assigned staff 2. Acts as client advocate in order to coordinate required services or to resolve crisis 3. Counsels clients in individual and group sessions to help them overcome dependencies, recover from illness, and adjust to life 4. Identify environmental impediments to client of patient progress through interviews and review of patient records 5. May administer and coordinate services and activities of a specialized program 6. Monitors, evaluates, and records client progress according to measurable goals described in treatment and care plan 7. Organizes support groups or counsel family members to assist them in understanding, dealing with, and supporting the client 8. Performs clinical assessments of the behavioral health needs of consumers presenting for services 9. Provides case management services related to identified treatment needs 10. Provides training to direct care staff in implementing behavior modification techniques and treatment plans 11. Refers client or family to community resources to assist in recovery from mental or physical illness and to provide access to services such as financial assistance, legal aid, housing, job placement or education

Qualifications

Master's degree in Social Work with license and Three years of postgraduate related experience, with at least one year being in a supervisory role

Preferred Qualifications

Must be Licensed in the State of Georgia either as a Licensed Clinical Social Worker (LCSW) or a Licensed Master Social Worker (LMSW). Must have a valid Driver's License from the state in which candidate resides and clear MVR /driving record. Must meet policy requirements for driving a State-owned vehicle: must be 21 years of age and have 3 years of driving experience. Must have experience supervising other social services staff.
