Program Assistant - Intake & Evaluation

Job Description

Under general supervision, performs a variety of clerical duties in support of the Intake & Evaluation staff.

Job Responsibilities include, but are not limited to:
  • Communication to providers and staff
  • Filing
  • Copying and scanning
  • Record keeping
  • Entering data from forms
  • Maintaining applicable records
  • Conduct comprehensive research using a variety of resources to generate reports and respond to requests
  • Review travel documents
  • Maintain a database to track new individuals in the waiver program
  • DMA-6 tracking
  • Maintain several databases and spreadsheets


High school diploma or GED and one year of related experience OR one year of vocational/technical school or college education OR one year at the lower level or at an equivalent position

Preferred Qualifications

Proficient with Microsoft Word and Excel
