Social Service Provider - Repeat Admissions Review (RAR) Designee
The Georgia Department of Behavioral Health and Developmental Disabilities (DBHDD), is seeking qualified candidates for the position of Social Service Provider - Repeat Admissions Review (RAR) Designee at Georgia Regional Hospital At Savannah (please visit www.garegionalsavannah.com for more information about the hospital).

Primary responsibility to perform Repeat Admissions Assessments, psychosocial assessments, treatment planning/monitoring and discharge planning for the Repeat Admissions Review patients upon their admission to the hospital. . Also provides counseling/therapy to clients as assigned or as clinically indicated. . Collects and compiles data for the Repeat Admissions Review system as assigned. . Attends Treatment Team meetings and provides consultation on discharge plans as requested from the assigned Team Social Worker. . Serves as a line Social Worker during emergency or staff shortages as assigned. . Provides basic level in-service training for hospital staff regarding the social services delivery system as assigned. . Serves a Lead Group Facilitator in the hospital Psychosocial Rehabilitation Program as assigned.

Master's degree in Social Work and eligibility to be licensed in the State of Georgia as a LMSW or LCSW.
Candidates with a Georgia LMSW or LCSW are preferred. Must have Master's in Social Work (MSW). Experience working with individuals in Adult Mental Health Inpatient setting. Must have excellent organizational skills, the ability to work in a fast-paced arena, and the ability to develop comprehensive discharge/ transition plans. Proficiency in psychosocial assessment, group facilitation, crisis intervention, treatment and discharge/transition planning. Experience in providing clinical supervision to staff and interns.
