Job Description

The Georgia Department of Behavioral Health and Developmental Disabilities (DBHDD) is seeking qualified candidates for MDS Nurse (WOW/Hourly) at Southwestern State Hospital in Thomasville, Georgia

Rose Haven is an inpatient unit which serves developmentally disabled individuals in an Intermediate Care Facility (ICF/MR) and a Skilled Nursing Facility.  For additional information, please visit our website at www.swsh.org and click on the Services link.

The Minimum Data Set (MDS) is part of the U.S. federally mandated process for clinical assessment of all residents in Medicare or Medicaid certified nursing homes. The MDS nurse will conduct a comprehensive assessment of each resident's (20 Beds) functional capabilities and helps nursing home staff identify health problems.

 MDS nurse will gather information on the facility's current patients for future assessment, including physical and mental states. The nurse will assess charts and communicate with healthcare teams to create applicable health care plans for their current and incoming residents. MDS nurses coordinate the delivery of care services to our residing residents. The MDS nurse will also observe and document the effectiveness of these services.  This nurse will also provide OBRA (Omnibus Reconciliation Act of 1987) assessments to ensure that the facility complies with Medicaid or Medicare standards. The MDS nurse may be required act as the main communicator between the insurance professionals and management officials at the facility.  The nurse will also be expected to assist the Director of Nursing as indicated.

A Current Georgia license to practice as a licensed practical nurse or registered professional nurse

Must obtain Safety Care certification by completion of new employee orientation (this certification must be maintained as a term and condition of employment).

Preferred Qualifications

·          MDS Certification

·          Experience with jRAVEN operating system

·          Previous work experience in a hospital setting

·          Georgia licensure as an RN

·          Experience working with developmental disabilities
